How to complete form SR01, part by part

How to complete SR01, part by part: what goes in each box, how many documents fit on a page and the mistakes that get an application sent back.

Updated 24 Oct 2026 · 9 min read · By Rayhaan Moughal, Sidekick Accounting

To complete SR01, fill in six parts: your details, what you want removed, the address, the documents, your printed name and a contact address. Then pay £34 per document through GOV.UK Pay and email the form with the payment reference to Companies House.

The form is short. The part that takes the time is Part 4, because you have to find every filing that shows your details and describe each one correctly. This guide follows the official SR01 guidance on GOV.UK. For the bigger picture, read what the SR01 form is and who can use it.

How to complete SR01: guidance part by part

  1. 01Part 1: your detailsYour full name, including any former names, your full date of birth and an email address. Companies House says the email helps avoid delays, so use one you check.
  2. 02Part 2: what to removeTick every box that applies: the day of your date of birth, your business occupation, your signature or your home address. Ticking several is fine.
  3. 03Part 3: the addressGive the home address to be removed. Only add a replacement correspondence address when you need one (see the next section).
  4. 04Part 4: the documentsOne row per filing: company number, company name, document title or filing type, and the date Companies House registered it. Three per page.
  5. 05Part 5: authenticationPrint your name. There's no signature and no company authentication code.
  6. 06Part 6: presenter detailsA postal address for contact. If Companies House rejects the application, it posts it back here.

If you want to tick day of birth, check the date first. It only works on documents filed before 10 October 2015, and the month and year stay public. We cover it in removing your date of birth.

Fill in SR01: do you need a replacement address?

Part 3 has two sections. The first is the home address you want gone. The second is a replacement, and most people leave it empty.

Do you need a replacement address?

Are you a current director, secretary, member or PSC at a live company?

Yes: Go to the next question.

No: Leave the replacement section blank. Resigned officers and dissolved companies don't need one.

Is the home address still your service (correspondence) address on the register?

Yes: Give a replacement correspondence address in Part 3, section 2.

No: Leave it blank. A home used only as a former registered office doesn't need a replacement, and Companies House hides part of it.

If the home address is a company's current registered office or its SAIL (single alternative inspection location), change it first with form AD01 or AD02. Companies House won't remove a live company's current registered office.

How to fill in the SR01 continuation page for Part 4

Part 4 is where most of the work sits. For each document you need four things: the company number, the company name, the document title or filing type, and the registration date.

Find them in the Companies House filing history for each company you've been involved with. The registration date is the date Companies House registered the filing, which can differ from the date on the document itself.

Here's how a completed row looks, using a made-up company:

A fictional Part 4 row. Each of the three slots on a page looks like this.

Each page holds three documents. For a fourth, you use an SR01 continuation page, which GOV.UK publishes as a separate PDF alongside the form. Fill one in for every further batch of three and send it with the main form.

Two limits apply. Only documents registered on or after 1 January 2003 qualify, and you can only list documents Companies House is able to suppress. Its list of suppressible documents is linked from the SR01 page. Signatures, for example, can't come off charge instruments, deeds or debentures, though they can come off 403a and 403b declarations. More on that in removing a signature.

If you'd rather not hunt through filing histories, our free scan reads every filing since 2003, including handwritten paper forms, and lists the documents to put in Part 4.

The AD01 trap

Pay the fee and send the form

Pay before you apply. The fee is £34 per document listed, paid through GOV.UK Pay. You'll get a payment reference at the end, and you must include it with your application.

Email the completed form and reference to dsr@companieshouse.gov.uk. That's the quicker route. You can post it with a cheque or postal order instead, but GOV.UK says that usually takes longer.

Why SR01 gets rejected, and how to avoid it

If you're wondering why an SR01 was rejected, it's nearly always one of these:

ReasonHow to avoid it
Listed the AD01 instead of the original filingList the filing that first gave your home as the registered office
No payment referencePay first, then attach the reference
Wrong fee£34 multiplied by every document, continuation pages included
Wrong document detailsCopy the number, title and registration date from the filing history
Document registered before 2003Check the registration date. Only 2003 onwards qualifies
Current registered office of a live companyChange it with AD01 first, then apply
Signature on a charge documentLeave it off. Those signatures can't be removed

Companies House returns the application to the Part 6 address, so you can correct it and send it again.

If you'd rather have someone else do the checking, our service files the application for you, and refunds the £34 on any document Companies House rejects. Pricing starts at £69 for the first document (VAT included).

Questions

Do I need to sign form SR01?

No. In Part 5 you type or print your name to authenticate the application and Companies House doesn't ask for a signature. You also don't need a company authentication code, because you're applying as an individual.

Why was my SR01 rejected?

The usual causes are a missing payment reference, the wrong fee, incorrect document details and listing the filing that moved a registered office away from your home instead of the original filing. Companies House posts a rejected application back to the address in Part 6. Fix the problem and apply again.

How many documents can I list on one SR01?

Part 4 holds three documents per page. If you need more, add an SR01 continuation page for each further batch of three. The fee is £34 per document listed, so five documents cost £170 whichever pages they sit on.

Do I have to give a replacement address?

Only if the home address you're removing is still your service or correspondence address in a live role, such as a current director. If you've resigned or the company is dissolved, leave the replacement section blank.

Can I email the SR01 or does it have to go by post?

You can do either. Email to dsr@companieshouse.gov.uk with your payment reference is the quicker route, and post (with a cheque or postal order) usually takes longer to process.
